Benefits and Coverage

Your Local Union and Participating Employer work together to provide you with the enclosed package of collectively bargained benefits through the Teamsters Benefit Trust (TBT).

We urge you to take the time to review this package and understand how your benefits work. You need to use your coverage wisely to get the most out of your benefits.

Here are a few important features:

Choice of Medical and Dental Options

You can enroll in the Indemnity Medical option (explained in this package) or one of the Health Maintenance Organization (HMO) options explained in the Comparison of Medical Benefits. You also have a choice of dental options explained in the Comparison of Dental Benefits.

Incentives to Use Certain Features

The Indemnity Medical option generally pays higher benefits when you use Preferred Provider Organization doctors and hospitals. The difference in benefits can be substantial. Be sure you know what your out-of-pocket costs may be when you or your family need medical care. Read your Summary of Coverage for specific details about your TBT benefits such as the deductible, copyments, benefit maximums and exclusions.

Enrollment Materials

New participants must fill out the TBT enrollment form in the enrollment materials folder and return it in the enclosed envelope immedidately. Your coverage will not begin until you return the form. You also need to return the Medical Option Form and Dental Option Form. If you choose coverage through an HMO, the HMO application is also needed.

Broad Package of Benefits

In addition to your medical and dental plan options, TBT provides other benefits including life and accidental death and dismemberment protection. You also have prescription drug and vision care benefits no matter which medical or dental options you choose.

All of your benefits are more fully explained in the Guide to Your Benefits and other materials inside. Your package also contains a supply of forms. From time to time, the Board of Trustees may change the benefit provisions of the Plan. Keep any notices of changes in this package so you can refer to them when needed.
